We currently have
an exciting peripatetic vacancy within our Integrated Hospital Discharge team,
the role will be primarily based at Wythenshawe Hospital .
The Hospital
Discharge Teams are integrated Health and Social Care teams that offer
partnership working to facilitate safe and timely discharges for patients to
leave hospital and return to their communities. During the current Covid-19
pandemic the hospital social work teams are providing professional support and
guidance to hospital trusts who are discharging patients with trusted assessments
to Discharge to Assess services either in the person’s own home or into 24 hour
care settings. Our hospital Social work
teams are assessing people after they have left hospital and planning for their
long term support needs.
We require an experienced
and confident Level 3 Social Worker to support the leadership of the Trafford
Social Care workforce based across our hospital discharge teams.
You will be
reporting to the Senior Practitioner and the Team Manager, and will be expected
to provide peer support and guidance to other Social Workers and Social Care
assessors in the team.
You will need a
recognised social work qualification and current Social Work England
registration. You will have at least 2 years’ experience of working in relevant
settings.
You will have
strong assessment and risk assessment skills, and the ability to be confident
in making professional recommendations.
You will be confident in providing verbal and written reports to a range
of professionals both within the Integrated Discharge Team, the wider hospital
environment and to senior management.
You will keep up
to date with the relevant regulations, policy and legislation relating to the
Care Act 2014, Mental Capacity Act 2005, and hospital discharge procedures.
