We require an experienced and confident Level 3 Social Worker to support the leadership of the Trafford Social Care workforce based across our hospital discharge teams.

You will be reporting to the Senior Practitioner and the Team Manager, and will be expected to provide peer support and guidance to other Social Workers and Social Care assessors in the team.

You will need a recognised social work qualification and current Social Work England registration. You will have at least 2 years’ experience of working in relevant settings.

You will have strong assessment and risk assessment skills, and the ability to be confident in making professional recommendations.  You will be confident in providing verbal and written reports to a range of professionals both within the Integrated Discharge Team, the wider hospital environment and to senior management.
