Social Care Assessor

We are recruiting a Social Care Assessors to join the Discharge to Assess Team/ Urgent Care Team on a part time basis with flexibility in working days Monday - Friday. Following the success of the Discharge to Assess model, which has seen a reduction in the number of long term decisions being made for individuals who are currently in hospital, we are looking for enthusiastic and motivated applicants looking to undertake a new challenge.  The service currently covers Discharge to Assess beds based in Ascot House and community providers and works closely with the CEC, SAMS, Care at Home and ICT services. The main area of work is to provide support to adults and / or their carers who may be eligible for assessment and care services under the Care Act 2014.  You will be required to arrange safe and timely discharges from Discharge to Assess facilities as well as SAMS, ICT, Care at Home and CEC within agreed timescales to ensure best possible outcome for people.

 

You will need to have relevant experience and knowledge around the role and responsibilities of a social care assessor. As this is a secondment opportunity transferrable skills will be taken into consideration.

 

You will manage a varied caseload, which will include undertaking support with accommodation and follow up reviews/ re-assessments. You will deliver exceptional social care to adult service users and their carers which enables them to make their own decisions. You will need to operate within an agreed performance management and budgetary framework. A significant part of your role will be to identify risks to the individual and respond appropriately ensuring any safeguarding issues are addressed.

 

Current Team hours are Monday to Friday 8:30-16:30 with flexibility on discussion with management around how the part time hours will best fit both the successful applicant and the team.  You will receive regular informal and professional supervision and will be part of a developing, enthusiastic team.

From 11th November 2021, all CQC registered care home workers and other visiting professionals will need to be fully vaccinated against COVID-19, unless they have a valid exemption. As one of our office bases associated with this role is based in a CQC registered care home you must have been fully vaccinated against COVID19 or be able to demonstrate medical exemption. Any offers of employment will be subject to proof of vaccination/exemption status.
