Trafford Older Adults CMHT are looking to recruit a Senior Practitioner Social Worker to join our team who holds the AMHP qualification, or is willing to complete AMHP training. Working within the Older Adults Community Mental Health Team in Trafford, the successful candidate will be based in a multi-disciplinary team managed by Greater Manchester Mental Health NHS Foundation Trust and will work alongside colleagues from nursing, occupational therapy and medical backgrounds.
The successful candidate will be a skilled and reflective social work practitioner with demonstrable experience of working with people with an organic or functional mental health diagnosis within a community setting.

The successful candidate will; care co-ordinate a caseload of service users, carry out rotational duty work and assess new referrals, take on responsibilities as the safeguarding lead for the service, , complete and support others to complete social care commissioning for a range of care packages, contribute to the out of hours weekend work on a rotational basis  and  actively contribute to a multidisciplinary approach to the management of the health and social care needs of the users of our service. The successful candidate will be familiar with the CPA process and report writing for Mental Health Review Tribunals. The role will include AMHP duties and being part of the AMHP rota.

You will be joining a highly supportive and motivated team with a strong commitment to learning and practice development. The successful candidate will provide clinical supervision to other members of the team and can also expect to receive regular clinical and managerial supervision.
