Trafford Local Care Organisation (Trafford Council and MFT) are recruiting an experienced Social worker to join their integrated Urgent care service, Wythenshawe hospital team.

 

The successful candidate will have significant experience of working with adults in need of health and social care support, hold a recognized social work qualification, and be registered with Social Work England.

 

The team is looking to appoint an individual who is motivated to challenge current ways of working and is enthusiastic to engage in adopting strengths based, person centered assessments of need.

 

A significant part of your role will be to engage in outcomes focused and preventative conversations which build on the strengths of people and their local networks. 

 

In addition you will be required to provide an effective response to safeguarding enquiries and support for people with impaired capacity to make decisions.

An exciting opportunity has arisen for a social worker to join Trafford Council as part of the Trafford Local Care Organisation. We are looking to recruit a social worker to join our urgent care service within the Wythenshawe hospital team.

 

About you

 

The successful candidate will require an understanding of the assessment and review process/function, incorporating knowledge of care & support planning and its purpose. Detailed understanding of the National eligibility criteria under the Care Act 2014 is essential and an awareness of how the domains are applied to determine eligibility for services.

 

You will need to have experience of working with adults in need of health and social care support. The team is looking to appoint an individual who is motivated to challenge current ways of working and is enthusiastic to engage in adopting strengths based, person centered assessments.

 

Your style and approach will reflect our EPIC values and behaviors. You will be committed to continually learning & improving the way you do things to ensure effective outcomes. You be able to work autonomously as required and as part of a wider multi-disciplinary team when necessary. You will be adaptable to change and the new ways of working that are being developed in line with the current hospital discharge process.

 

You must possess good communication skills and be able to work proactively with a range of adults, applying a clear understanding of the legislative framework for Adult Social Care. In addition you will be required to provide an effective response to safeguarding enquiries in line with both the Care Act and local policy and procedures.

 

You will need to hold a relevant social work qualification, be registered with Social Work England and have already completed the ASYE programme. An enhanced DBS check is required. There is an expectation that you will have relevant experience and knowledge around the role and responsibilities of a social worker.

You will receive regular informal and professional supervision and will be part of a developing, enthusiastic team.

We are recruiting a Social Worker to join our Urgent care Social Work team (Wythenshawe hospital). We are looking for enthusiastic and motivated applicants who will thrive in a busy work environment. You will be required to arrange safe and timely discharges both from an acute and community setting as per the Discharge to assess model of assessment. Assessments will need to be completed within agreed timescales to ensure best possible outcomes for people.

 

You will manage a varied caseload, which will include capacity assessments, best interest decision making, Care Act assessments, safeguarding investigations, and being part of a duty rota. You will deliver exceptional social care to the adults we are supporting and their carers and promote choice and control as much as possible. You will need to operate within an agreed performance management and budgetary framework.
